| Adult Training Services
A variety of programs and services are available to adults with developmental disabilities aged 16 years and older who no longer participate in a secondary educational program. Facility Based Sheltered Work - A short term or long term work oriented program for the purpose of training in work skills while being provided remunerative employment. Transportaiton - Transportation is available from the individuals residence to the site for training. Habilitation Training - A program of goal centered activities designed to assist the individual to participate more effectively in all phases of daily living, especially in integrated settings. Training can include, but is not limited to, mobility, nutrition, recreation, leisure, social and interpersonal dkills, decision making, self-advocacy, academic, motor and perceptual skills and communicaition skills. Senior Day -- A program for adults with developmental disablities over fifty years of age who no longer can pariticipate in or desire to participate in vocational or work programs but desire a day program revolving around community and other programed activities. Semi-Independent Living Program - This is program of residential support for those living on their own. Training and support is provided in money management, personal safety, nutrition, leisure, community living and other service needs of the individual. New Horizons also provides services through the Medicaid Waiver. The Medicaid Waiver program is part ofthe Indiana Medicaid Program which is a Federal and State medical assistnace program. The waiver was designed to support those residing in their home and community whose level of care might otherwise require care in an institution or other type of facility. Application for the Home and Community Based Medicaid Waiver Services is through the local Council on Aging. In the service area of New Horizons it is Area 12, phone 812-432-5215. The services provided by New Horizons under this program are: Residential Based Habilitation - These are integrated living support services designed to assist the individual to reside successfully in their own home through a program of adult daily living skills training and behavior management Personal Assistance - These are services that provide necessary help to meet daily living needs and ensure adequate functioning in an independent living arrangement or within the family. Day Habilitation-- These services are regularly scheduled activities apart from an individual's home, which focus on training to enhance social development and skill development in daily living and community living tasks.
